Privacy and safety notice

This app stores medical log data, images, uploaded documents, and notes locally on this device/browser using localStorage. It does not create a server database and does not send your log data to Netlify or Google Sheets unless you export, email, copy, fax through an outside service, or otherwise share it.

Exported CSV, PDF, JSON, images, and documents may contain sensitive health information. Only export/share when you intend to, and protect exported files.

This tool is for personal organization and sharing. It does not provide medical advice and does not confirm medication appropriateness, dosing, diagnosis accuracy, insurance coverage, pharmacy accuracy, or adherence safety.

Only nonblank profile fields are included in PDF/CSV/JSON exports.

Vitals, Height, and Weight

BMI is calculated automatically from height and weight. Reference ranges from the source form: Systolic 90–140 mmHg, Diastolic 40–90 mmHg, Pulse 35–150 bpm.

Major Events Log

Major events are usually historical. When you select a start date, the stop date defaults to the same date unless you change it.

Audit Trail

Each profile has its own audit trail; this shows changes for the active profile made in this browser.

Export

Exports may contain sensitive health information.

Email Summary PDF first opens the print dialog so you can save the Summary as a PDF (the audit trail is never included), then opens a blank email draft — attach the saved PDF manually. Browsers cannot attach files automatically from a static site.

Import / Transfer

Importing replaces or adds local data in this browser. Audit history is included if the JSON backup contains it.
